FORA Telehealth Gateway

The Telehealth Gateway device enables users to upload measurement data from any meter to the FORA cloud-based telemonitoring platform. By connecting via Bluetooth the measuring device, the Gateway allows for easy and seamless transmission.

Simply test using any of the ForaCare devices, upload measurements to the Gateway, and done! Clinicians, patients, and family members alike are then able to view and track the patient’s results and overall status via the FORA 24/7 HealthView telemonitoring platform.

  • Easy- to-use cellular device: Plug it in for automatic data transfer from FORA devices to the 24/7 HealthView telemonitoring platform
  • Built-in drivers for FORA devices: Quickly connect to FORA devices for automatic uploads
  • LED and sound indicators: Two short beeps and a green light under WSN indicate a successful transmission
  • AC power adapter: Plug it in to a power source and leave it turned on

The FORA Gateway is a cellular device that enables patients to effortlessly transmit data from their devices to the FORA 24/7 HealthView telemonitoring platform.

Patients simply plug the Gateway in to a power source, turn it on, and take a measurement. When the result appears on the device, the data transmits to the Gateway via Bluetooth and uploads it to the telemonitoring HealthView platform for review.

Remotely monitoring patient data in real time allows healthcare professionals to prevent health complications and reduce hospital readmissions, lower costs, and increase testing compliance.

New remote monitoring reimbursement options

Because the Gateway allows patients to upload their results from a FORA device to the 24/7 HealthView telemonitoring platform, you can receive reimbursement for remote patient monitoring. There are a couple of different reimbursement options depending on what your patient is being monitored for.

For remote monitoring of blood pressure, use reimbursement codes 99473 and 99474. Complete the following actions to receive reimbursement for each code.

  • 99473 Onboard a new patient into ForaCare’s remote patient monitoring program and provide patient education on a FORA blood pressure monitoring device such as the P20b 
  • 99474 Patients are required to take an average blood pressure measurement with their FORA device (consisting of two readings, one minute apart, twice daily), which are automatically uploaded to the 24/7 HealthView Telehealth System. Review the patient data on the 24/7 HealthView system and communicate a treatment plan to the patient 

For remote monitoring of blood glucose, pulse oxygen, temperature, or weight, use reimbursement codes 99453, 99454, 99457, and 99458. Complete the following actions to receive reimbursement for each code.

  • 99453 Onboard a new patient into ForaCare’s remote patient monitoring program and provide patient education on the Gateway, along with whichever FORA device fit their needs such as the Premium V10 BLE glucometer, TN’G SpO2 pulse oximeter, IR20b thermometer, or TN’G Scale 550
  • 99454 Supply the FORA monitoring device and the Gateway to the patient and receive daily recordings
  • 99457 Spend 20 minutes remotely monitoring the patient on the 24/7 HealthView system and interact with the patient via phone, text, or email
  • 99458 Continue to remotely monitor the patient for 20 additional minutes on the 24/7 HealthView system and interact with the patient via phone, text, or email
