ForaCare (“FORA”) does care about the privacy of our users. The Privacy Policy applies to the service of FORA TeleHealth or iFora apps (“Services”). This Private Policy will show the scope of personal information to be collected and the process and purpose we will use for your personal information.

To ensure the safety while you use the Services, we would like to explain our Privacy Policy to you. Please read the following content carefully to protect your rights and interests.

 

1. The way we process and collect the Personal Data

FORA will process and collect the Personal Data depending on whether you use telehealth system and the variety of access.

  • Not use of telehealth system

A. We will collect personal information (e.g. birthday, age, gender, height…) to identify the user.

B. Depends on your health plan, we will collect your physiological data (ex: blood glucose, blood pressure, temperature, weight, Blood oxygen concentration) to provide APP services.

C. The above data collection is only stored in your APP and will not be used for additional processing and storage.

  •  Use of telehealth system

If you are a medical/health professional,

A. When you set up an account to register for our Services, we may collect, process and use your personal information including your name, E-mail address, set account and password, time zone and other information.

B. The Services will use your personal information under the situations below:

(i). creating new accounts, providing, renewing, maintaining Services.

(ii). support for Services

If you ask for the technical support of the Service, we will provide support to you or the account(s).

(iii). notice of alteration or event

If there is any alteration or event of the Service will affect your interest, we will send a notice to you or the account(s).

(iv). safety security

We will save and redundancy your personal information under safety security under reasonable and commercial practice.

If you are a user,

A. When you agree to use our Service or you authorize a medical/health professional to use our Services, we may collect, process and use your personal information, including your name, birth date, gender, height, weight, E-mail address, present address and other information, to provide the Service.

B. The Services will use your personal information under the situation that

(i). transferring the physiological data

Depends on your health plan, we will collect your physiological data (ex: blood glucose, blood pressure, temperature, weight, Blood oxygen concentration) and transfer them to your account. Your authorized medical/health professional will be able to access your physiological data in order to provide health care services.

(ii). creating new accounts, providing, renewing, maintaining Services.

(iii). support for Services

If you ask for the technical support of the Service, we will provide support to you or the account(s).

(iv). notice of alteration or event

If there is any alteration or event of the Service will affect your interest, we will send a notice to you or the account(s).

(v). safety security

We will save and redundancy your personal information under safety security under reasonable and commercial practice.

2. Cookies or any other mechanisms

In order for the system to recognize the user’s connection, the system will record the user’s behavior in the form of cookies or other similar technologies as authorization management for using the service, monitoring of abnormal behavior, solving technical problems, and providing individualized service content. These cookies are necessary, and with them you can browse the entire website and use its functions, such as accessing personal health information. Without these cookies, you may not be allowed to use the Services.

3. Data transferring and sharing

For providing, improving and maintaining the Service, FORA may share your personal information with your medical/health professional. Your medical/health professional may access or process your information to provide professional services.

4. Term of storage

FORA processes and stores your personal data according to specific purposes only when you use the Service and under your consent. If you do not continue to use the Service, we will cease to provide the Services and delete your account and personal data according to your written request or your medical/health professional written request.

5. Safety of personal information

A. We are dedicated to protect the security of personal information and has a security management mechanism under reasonable and commercial practice. We use various security technologies and procedures to protect your personal data to prevent unauthorized access, use or disclosure.

B. You should keep your account password properly. After using all the Service functions provided by this system, you must remember to log out of the account. If you share the computer with others or use a public computer, remember to close the browser window to prevent others read your information or letter.

6. Right of personal information

A. Users can exercise the rights of the following ways:

(i). You can inquire, receive personal information about you, or request a copy, and we may charge necessary costs.

(ii). You can request to mark or delete incorrect data.

(iii). You may request to stop collecting, processing or using and request to delete all your personal data.

(iv). You may request to bring your personal data out

(v). The right to withdraw this consent can be revoked at any time

(vi). If you do not satisfy our Service, we will try our best to solve your concern. You are entitled to report or appeal to the proper Supervisory Authority.

7. Protection of privacy of child

We do not specifically collect children’s personal data. Since it is important for us to protect children when they use the Internet, we recommend that all parents and legal guardians teach children how to use the Internet safely and responsibly. Without the parents’ consent, children shall not be allowed to transfer their personal data to FORA.

For business needs, updated of statutory regulations and technological developments, we will update the Privacy Policy from time to time, and publish the latest version on the website. Please refer to this policy at any time to protect your rights and interests. If any alternations to this policy will affect your rights or interests, you will be notified gradually before the effective date.
