Improving Care Beyond the Clinic with ForaCare’s RPM Solution
Background:
While attending the American Association of Nurse Practitioner conference in San Diego, our team connected with a Nurse Practitioner (NP) from the Southern Nevada Care Center. The center focuses on providing primary care to a growing population of senior patients across Southern Nevada and was seeking an effective way to manage chronic conditions at scale. While familiar with Remote Patient Monitoring (RPM), the practice had not yet implemented a program.
Growth and Implementation:
