Improving Chronic Care Beyond the Clinic with ForaCare’s RPM
Background:
While attending the American Association of Nurse Practitioner conference in San Diego, our team connected with a Nurse Practitioner (NP) from the Southern Nevada Care Center. The center focuses on providing primary care to a growing population of senior patients across Southern Nevada and was seeking an effective way to manage chronic conditions at scale. While familiar with Remote Patient Monitoring (RPM), the practice had not yet implemented a program.
Growth and Implementation:
After evaluating ForaCare’s cellular-enabled devices and 24/7 HealthView platform, the NP launched the RPM program with a select group of patients. The program quickly demonstrated value by improving patient engagement, enabling consistent at-home monitoring, and providing the care team with reliable, real-time health data. With greater visibility into patient trends and the ability to intervene sooner, the practice saw meaningful improvements in clinical outcomes. Additionally, Medicare reimbursement for RPM services created a sustainable new revenue stream, reinforcing both the clinical and financial benefits of the program.
Outcome:
As confidence in the program grew, the NP expanded the RPM program across a broader patient population. With streamlined workflows, automated documentation, and strong patient adherence, RPM became a scalable and integral part of the practice. Today, ForaCare supports a more proactive, data-driven approach to chronic care—helping the Southern Nevada Care Center deliver better outcomes while improving operational efficiency and long-term practice growth.
