ForaCare to Feature Remote Patient Monitoring Solution at Annual CAHP Conference, October 21–23

The ForaCare team is traveling to Palm Desert next week for the CAHP Annual Conference to present our remote patient monitoring solution—a proven technology that provides increased access to care while decreasing healthcare delivery costs.

Incorporating ForaCare’s remote patient monitoring solution has already led to positive health outcomes: reduced hospital readmissions, fewer complications, and improved quality of life including decreases in HbA1c levels and lower mortality rates in CHF patients.

Patients receive FORA® devices for chronic disease management such as diabetes, hypertension, and congestive heart failure. Once patients take a measurement, the data is uploaded to the FORA 24/7 HealthView Telehealth System, a cloud-based platform for healthcare professionals to keep track of patient trends in real time, manage and adjust medications, and intervene in an emergency.

Our easy-to-use FORA devices can measure blood glucose, blood pressure, weight, pulse oxygen, and temperature.

ForaCare to Attend AHCA/NCAL Convention & Expo, October 13–16

Our ForaCare superheroes are flying to the 70th annual AHCA/NCAL Convention & Expo in Orlando, Florida to showcase the FORA® Mobile Care Station as an answer to some of the challenges long-term care providers face.

The Mobile Care Station provides a more efficient means to measure vitals and monitor chronic care patients because of its portability and accuracy.

One rollable cart holds five medical technologically advanced devices used to measure blood glucose, blood pressure, temperature, pulse oxygen, and weight so nursing staff can easily move between patients. Since the measurements automatically populate into the iFORA CS (PCC) app and then integrate to the EHR, manual entries are eliminated, which saves time and reduces human error.

Most importantly, the Mobile Care Station meets or exceeds all HIPAA requirements because of its complex encryption and authentication measures.

RPM Company ForaCare Named Among CIO Applications Top 10 Remote Monitoring Solution Providers of 2019

ForaCare Inc., a remote patient monitoring (RPM) company that combines innovative technologies with accurate medical devices, was recognized among CIO Applications Top 10 Remote Monitoring Solution Providers of 2019.

RPM technology gives physicians the tools to better manage patients’ chronic conditions such as hypertension, diabetes, CHF, and COPD. FORA® devices automatically upload patient measurements to the 24/7 HealthView Telehealth System, where physicians are able to review the information in real time and help prevent complications.

ForaCare’s RPM solution is unique because we’re a one-stop-shop for healthcare providers, said Adrian Alexander, Senior Account Manager for ForaCare, in an interview with a representative from CIO Applications. As opposed to the competitors, no middle man is required since ForaCare’s lab develops the software and all the devices are manufactured in-house.

Additionally, ForaCare’s RPM solution is able to easily integrate with other electronic health records and the HIPAA-compliant software and devices provide strong encryptions for patient data.

“We also invest heavily in R&D to come up with new products that are simple, easier to use, and more efficient. This is one of the major factors that steer us ahead of the competition, other than our quick response time to the clients’ problems,” Alexander said.

Introducing the New FORA A1C100 Analyzer

ForaCare is excited to introduce the new FORA® A1C100 Hemoglobin A1c Analyzer to better serve medical professionals in diagnosing and monitoring diabetes.

With just a tiny sample volume (<0.2 µl) of blood, physicians will receive high-quality HbA1c and eAG results ranging from 4% to 16% in less than 6 minutes using the FORA A1C100. Plus, since the device requires no sample pretreatment and no pipetting, its fully automatic operation reduces human error.

It’s as easy as 1-2-3:

1. Collect the blood sample.

2. Insert the sample in the capillary.

3. Insert the cartridge into the A1C100 Analyzer and gently pull the tab.

Additional Features:

  • Upload results to a computer via a USB flash drive
  • Embedded thermal printer easily provides patients a copy of their results
  • Up to 10,000 results stored
  • Insert barcode scanner or keyboard in USB port for safer and faster patient ID entry
  • Easy to store and carry because of its small size

New Features Added to the FORA 24/7 HealthView

ForaCare has recently completed an update of the FORA® 24/7 HealthView Telehealth System adding several new features providing additional remote patient monitoring benefits for physicians or caregivers.

The three new main functions include:

  • Action log
  • Billing reports
  • Messaging

Each patient profile now has an action log page. This allows physicians to record their actions interacting with each patient and the minutes spent taking that action. For reimbursement purposes, group managers (or physicians) can run billing reports related to remote patient monitoring codes 99453, 99454, 99457, and 99458.

Plus, the messaging system has been updated for better communication. Physicians and patients can interact through messages and questionnaires within the 24/7 HealthView system.

Get Started with ForaCare’s Pilot Program for Better Chronic Disease Management

In as little as 30 days, healthcare providers and health plans interested in remotely monitoring their patients can begin a pilot program with ForaCare.

With a variety of FORA® devices available, ForaCare has a range of solutions that will fit anyone’s needs—whether managing diabetes, hypertension or congestive heart failure. After defining an end goal, we’re able to help you determine which specific FORA devices are right for your particular pilot program.

24/7 HealthView Provides Useful Tools for Physicians to Remotely Monitor Patients

The FORA® 24/7 HealthView Telehealth System is an online platform that empowers healthcare professionals and caregivers to remotely monitor their chronic care patients and provide better health outcomes.

The HIPAA-compliant cloud-based system can safely be accessible anywhere, anytime. The best benefit the 24/7 HealthView System provides is how abnormal data shows up on the dashboard in either red or orange so physicians can review all out-of-range patient data at a glance. This allows physicians to identify high-risk patients and intervene in an emergency.

Features:

  • Alerts—Physicians are immediately alerted to patient abnormalities by auto-generated emails.
  • Connectivity—All FORA devices, which measure blood glucose, blood ketone, blood pressure, temperature, pulse oxygen and weight, connect to the 24/7 HealthView Telehealth System via Bluetooth or cellular.
  • Flexible reporting—Generate individual or group reports as pie charts, trend graphs and scatter plots.
  • Messaging system—Physicians can send messages to patients reminding them to take their measurements and medications as well as recommendations for diet and exercise, which helps with patient compliance.
  • Multi-level access—Group manager, caregiver or clinician, and the patient can each access the platform with varying levels of account management.

And soon new features will be coming. Since remote patient monitoring is now reimbursable because of the three new reimbursement codes created in the 2019 Medicare fee schedule of Chronic Care Remote Physiologic Monitoring, the 24/7 HealthView System will be updated to reflect that billing information. Plus physicians will be able to save patient insurance information.

ForaCare also provides the training needed for you to get started with the 24/7 HealthView Telehealth System, including helping with integration onto the platform.
